Our family practice is located between Medford and Grants Pass in the town of Rogue River. Our clinic is recognized by the State of Oregon as a Patient Centered Primary Care Home, which means we provide comprehensive, team-based, coordinated, care focused on quality and safety. With our medical staff of physicians, a physician assistant (PA) and a family nurse practitioner (FNP)—supported by 14 friendly and compassionate staff members—we are your first stop for illnesses, chronic conditions and injuries.

Brian J. Mateja, DO

Dr. Brian Mateja has been practicing medicine since 2005. He and his wife Erin have two sons, and, when not working, he enjoys skiing, gardening and cooking.

Heather Merlo-Grifantini, MD

Dr. Heather Merlo has been practicing family medicine since 2011. She enjoys spending time with her husband, Carlo, and their three children, camping, skiing, and cooking.

Cora Haze, PA-C, NRCME, BCHS

Cora Haze has been practicing since 2003. She and her husband David have 5 children and 4 grandchildren. In her spare time she enjoys spending time with her family, horseback riding and gardening.

David Ray, FNPDO

David Ray began his practice as a Family Nurse Practitioner in 2014. He lives in Grants Pass with his wife and four children. He loves the outdoors and is very active in the community.
